HIPAA notice
RPM services involve the collection and review of physiologic data. When you are enrolled as a patient, your information is handled according to applicable privacy and security requirements.
You will receive (or have access to) the practice Notice of Privacy Practices (NPP) during enrollment. Requests for records, corrections, or restrictions should be directed to the practice contact number.
