1) Eligibility
We confirm clinical fit and payer requirements. We coordinate with you and/or your referring clinician.
2) Consent & enrollment
We obtain RPM consent and explain how monitoring, communication, and escalation work.
3) Device setup
Devices are configured and paired (as needed). You receive instructions for correct measurement technique.
4) Daily/weekly readings
You record readings on the recommended schedule. Data is transmitted or entered per device capability.
5) Clinical review
Our care team reviews trends and documented alerts. Actions are recorded in the clinical record.
6) Escalation
If thresholds are crossed, we follow the escalation pathway: education, outreach, clinician notification, or higher-level care guidance.
